Details of Experience:
Falling at home after her legs “gave way” caused Peggy T. to seek medical attention. Following this sudden onset of weakness, caring for herself and walking became an impossibility. She presented in the ER with pain resulting from a fall and generalized weakness. A need to lose weight was thought to be a contributor to her condition. Subacute, short-term rehabilitation was recommended by her doctor and hospital case management staff. Due to prior experiences of unmet expectations with other short-term care communities, she did not consider this an option. After much persuasion, she agreed to give Creekside Center for Rehabilitation and Healing a try. 

Upon admission, Peggy was warmly welcomed and evaluated by members of our clinical and rehabilitation team. The need for physical and occupational therapy to increase strength was evident. Initially, she was dependent on staff to sit or stand and required moderate to maximum assistance with activities of daily living. Short and long-term rehab goals were set. A care plan was established following the resident’s wishes to regain her independence to return home safely. Her Concierge visited to ensure ongoing satisfaction.

Within the eight weeks, Peggy has been with us, and she has made significant improvements. She said she imagined herself walking before she was able to do so. This helped her maintain confidence. Her innate desire for independence was also counted as a plus in her progression. She expresses much gratitude to her therapists, who consistently gave her the extra encouragement she needed and helped her enjoy her therapy. She also mentioned, “When she felt down, the staff showed concern and lifted my spirits.”  This, according to Peggy, has made a lot of difference. In her spare time, she enjoys watching TV, reading and appreciates the staff painting her nails. Her favorite memory during her stay was watching the team relay races during nursing home week. She was also entertained by the resident and staff dance called “dancing through the decades,” where participants were dressed according to their favorite era. 

She will be discharged with the ability to walk with modified independence and perform most activities of daily living without the need for assistance. Her weight loss goal has been met, and she believes this has supported further improvement. When I questioned Peggy about her overall experience, her rating was a 10 out of 10. Her satisfaction level greatly exceeded her expectations. Now, she is looking forward to returning home to the place she calls “her little abode,” where she will begin home health therapy.
