How did this patient hear about Creekside Center? Case manager at the hospital
Details of Experience:
Ms. E Haley came to the Creekside community from Skyline Medical Center. She arrived on the evening of November 12, 2025, following hospitalization for a displaced comminuted fracture of the shaft of the left femur. In simpler terms, Ms. Haley admitted with a serious thighbone fracture that required her to remain non-weight bearing on her left leg.
Despite the unexpected injury, Ms. Haley arrived with a clear goal: to regain her strength and return home walking as independently as she always had. Upon admission, she was welcomed by Creekside’s interdisciplinary team, including nursing, therapy, social services, recreation, and the Concierge Director, each of whom played a role in supporting her recovery journey. Early therapy evaluations confirmed that her non-weight-bearing status would make progress challenging. Even so, Ms. Haley approached rehabilitation with determination and resilience. Day after day, she showed up to therapy ready to work, encouraged by the rehabilitation team and motivated by her vision of returning home.
Ms. Haley quickly became a bright presence within the community. In the therapy gym, her humor and positive attitude lifted the spirits of those around her while she worked diligently toward her goals. Outside of scheduled sessions, she continued practicing exercises independently in her room, demonstrating her commitment to healing and progress. Between therapy sessions, Ms. Haley could often be found in the community lobby, enjoying a warm cup of coffee, sharing stories about her family and beloved cat, and offering friendship and encouragement to fellow patients and team members. Her kindness and optimism became a meaningful part of daily life at Creekside.
Through perseverance, support, and consistent rehabilitation, Ms. Haley achieved remarkable progress. By the end of her stay, she was able to ambulate independently and safely climb approximately ten stairs! These are significant milestones that brought her closer to returning home. Ms. Haley expressed heartfelt appreciation for several team members who made a lasting impact on her experience, including Certified Nursing Assistants (CNAs) Valencia and Dani, Licensed Practical Nurse (LPN) Joseph, Concierge Director Jade, and Receptionist Gwen.
After 58 days of dedication and recovery, Ms. Haley was successfully discharged home on January 9, 2026. Creekside celebrates the progress she achieved and wishes her continued health, happiness, and independence in the next chapter of her journey.
